Abstract

The study provides clinicians and health administrators on how the application of low technology plays a significant role in health to intervene the common health problem that could worsen the heart after index hospitalization. In our primary study, a significant effect in reducing hospital readmission, self-efficacy and quality of life among community dwelling older people living with chronic diseases especially chronic heart disease (CHD). A secondary analysis of the care process of telephone follow-up of older people with CHD conducted. Common signs and symptoms and their related interventions shed some light on the development of an efficient telephonic assessment algorithm.
